In the instructions for I-140 it says to write none or "N/A" when necessary
So if we dont have a middle name:
1.) We write "none" in the boxes where it asks for a middle name
2.) In the part that wants the information for spouse and children it has given a format first/middle/last
so if there is no middle name, if my name is Lisa Smith for instance I would write: Lisa/none/Smith
is this correct?

Absolutely. If you dont have middle name, just say "N/A or none". My wife and daughter both did not have their middle names, I did the same. I got my green card (through EB2-NIW) last week in 3.5 months after I applied my concurrent applications. Though I got RFE for some minor issue due to not sending the ETA-9089 form, the whole process went very quickly.
