I-130 Instructions Regarding G-325A For Petitioner

Can you guys help me clear the confusion on this instruction piece. Does this mean I only need to just add my name and sign it or do I still need to fill in the information on the G-325A form that are not on the I-130?
Per I-130 Instructions:
D. A completed and signed G-325A Form for you (Petitioner) and Husband or Wife. Except for your name and signature, you do not have to repeat on Form G-325A the information given on your I-130 form.

Also, on my G-325A form, the question regarding my address overseas over a year, I have not lived overseas since I moved here 20 years ago. Do I need to list the last address I lived in 1991?

Thanks
