I-693 Vaccination

Today I took my wife for the Medical Examination and Vaccination Record - to complete I-693.
Going through the forum and the USCIS site - I came to conclusion that we need the following Vaccination Requirements: 27 through 49 years

  1. i. Td or Tdap- tetanus and diphtheria(if DT, DTP or DTaP) - NEEDED
  2. ii. MMR -
Varicella (chicken pox) - this is not required

and
  1. a. Tuberculosis – (TB) done
  2. b. Blood test for Syphilis

However the civil surgeon was not confident about the Polio vaccination and she requested us to provide proof. Does every one will have to carry their previous Polio report and is this mandatory?

Specify Vaccine: OPV/IPV: Date Received
Vaccine Given vs Completed Series vs Waiver to be requested from USCIS(Blanket/Not Medically Appropriate)
The doctor doesnt have an answer and asked me to contact the USCIS - since we dont have the documentation to show proof , not sure how to approach this. normally if the doctor gives Not medically approved - Not age appropriate - will USCIS accept it? For my GC i never gave the polio proof. Any idea guys..

I think based on this - we dont need to get the Polio - as its Not age appropriate - based on the CDC listing below
