I am a naturalized citizen. I want to now sponsor my mother. One of the papers I need to submit is my birth certificate. My birth certificate has the name of my dad (and not of my mom) - This is a school issued certificate that I used during my own GC processing.
What are my options to establish my relationship with my mother? My (canceled) Indian passport lists my mothers name. Would that suffice? Would my moms marriage certificate to my dad in conjunction with my birth certificate work*? Should I request my elementary school to issue me a new certificate with the name of my mother (per their records)?
Or would an affidavit for the birth certificate work best? In that case, can someone point me to the language of the affidavit?

A school issued document is not considered a birth certificate. You should obtain a birth certificate from the local authority of the place where you were born. A city muncipality or village panchayat can provide you a birth certificate.
If your birth was not registered, then you wont be able to get a birth certificate. In that case, a letter from proper authority stating that your birth record was not found will be needed. Along with such a letter you will also need two affidavits from close relatives who were aware of your birth. The letters should have your full name and full names of both your parents.
You must have obtained such letters when you got your green card.
I dont remember getting any affidavit for my GC. But let me see if I can get something issued from the local authority. The problem is with the red tape, this may be a very tedious process. Thanks for your reply.

Thanks for your reply. You dont have a choice - the letter of non availability is a MUST in absence of a birth certificate.
In absence of Birth Certificate for sponsoring my mother
