2nd EAD - wrong dates
Hello,
Recently, I received my 2nd EAD (about 3 days ago), however the dates on the new card are same as my previous (1st EAD card-current EAD card). Luckily i just noticed it. My question is how do i go about correcting it?
Do i just call USCIS customer service to have the dates corrected or should I take a infopass and contact the IO at DO. I am extremely nervous as we are closing on our house in 15 days and wanted to have the 2nd EAD in my hand.
Please advise on my next steps. MY timeline is as below:

When I called USCIS customer service, they want me to fill out another I-765 form and return the card they sent me. If i send the form and card back to USCIS, would I be still subject to 90 day rule? Will DO be able to issue a new EAD card with correct dates? Taken Infopass appointment for next week.
Does anybody have experience with error in their EAD card and how did they go about in correcting the mistake?
Please advise.
As I was told to fill out new I-765 form and mail the EAD card back in, I went to USCIS website to print the I-765 form. However, there seems to be an error on the form, every time I try to open the form. Is anyone having the same problem.
What is the other source of procuring this I-765 form. Please help as I urgently need to mail in this form.
