I am on a J-1 visa and I got my waiver regarding two-year foreign residence requirements.
I got married to an US citizen.
We live in Maryland.

Have you received public assistance in the United States from any source, including the United States government or any state,county, city or municipality (other than emergency medical treatment), or are you likely to receive public assistance in thefuture?

As I mentioned I am on a J-1 visa, I am working at a government department as an exchange visitor.

Q1: "Baltimore, MD". Vermont service center wont be handling your case, dont mail it there. Mail it to the Chicago Lockbox as mentioned on the front of the I-485

What visa did you arrive here on?

Depending on that, you may not have a Alien number. If you do it will be written on the back of your I-94, a ten digit number starting with an "A".

Most likely though you dont have one so should write "None" where it asks you for it.
