I have some questions about Section C question #17 and #21.

How should I answer question #17 "List husband / wife and all children of your relative".

If the beneficiary is my wife, who should I list the names under question #17? We do not have any childred together.

In question #21 If filing for your husband/wife, give last address at which you lived togher. (Include street, city, province, if any, and country).

she lives in Taiwan and I live in US. Even thought we will be getting married in Taiwan, but we never "lived" together. However, I do stay in her house when I visit her, and I plan to either stay in her house, or in hotel by the time we are going to get married in Taiwan.

What address should I put for question #21? Must Question #21 be answered?

If you want "any helps...to be helpful" I suggest that you should post the full question not only the question number, cause if you go by number is kind of difficult for us to look for the forms and find out what exactly youre looking for.

I think you should put only yourself as husband and thats it.

In question #21 If filing for your husband/wife, give last address at which you lived togher. (Include street, city, province, if any, and country).

The correct and honest answer is: NONE
